Very interesting about the promise of an NEC drug…I am very optimistic about the new wave of probiotic pharma potential for GI and GU microbiome modulation

I agree! I was disappointed by the AAP's policy decision. I understand why - I actually took care of a baby who died of a probiotic -related illness (contaminated batch). It was a horrific and preventable tragedy. If more regulation and tighter manufacturing means less risk and wider adoption, I am all for it!

I know a lot less (eg nothing!) about GU microbiome modulation, but boy does that seem like a great arena for improvement. What are the targets? UTI prevention?

yes, and there is data pointing to rise in kidney stones in children, teen girls, and women under 40; low calcium, high protein diets as well- I plan to write about this. Perhaps you are interested in a collaboration
